Lupus Mastitis in the Male Breast Mimicking Inflammatory Carcinoma

Abstract: A 42-year-old man presented with a moderate and diffuse enlargement of the left breast. The skin presented as a red and scaly patch, centered in the nipple (Fig. 1). Laboratory parameters, including IgG and IgM, anti-Borrelia burgdorferi antibodies, and luetic determinations by rapid plasma reagent (RPR), were normal.Histopathologic study demonstrated the typical characteristics of lupus profundus: a mixed lobular and septal lymphocytic panniculitis (Fig. 2), some lymphoid follicles in the infiltrate, lymphocy… Show more

“…7,10 Immunohistochemistry has been occasionally performed on these lesions primarily to exclude the possibility of a low-grade lymphoma. Results have shown a mixed population of CD20 + B-lymphocytes and predominantly CD3 + and CD4 + T-lymphocytes 8,12,14 with polyclonal plasma cells. 8 These are concordant with the results from case 1 in this article.…”

“…8 These are concordant with the results from case 1 in this article. In one report, polymerase chain reaction was performed showing polyclonal rearrangement of IgH and T-cell receptor-g. 12 In general, the histologic findings of LP are considered sufficiently diagnostic to allow for an accurate diagnosis. Clinical history of lupus and the presence of overlying epidermal and dermal changes of lupus are also helpful diagnostically.…”

“…Pathologically, IBC is characterized by extensive lympho-vascular invasion and involvement of dermal lymphatics [6][7][8][9][10]. The clinical diagnosis of IBC, however, is often very difficult, since a number of breast and other lesions morphologically simulate IBC [11][12][13]. Although a number of molecules, including carbonic anhydrase IX, epidermal growth factor receptors, transmembrane mucin glycoprotein, Wingless-Type 1 induced secreted protein 3, c-erb-B2, and E-cadherin, are aberrantly expressed in IBC, none of these markers can specifically or reliably identify the IBC cells metastasized to lymph nodes or cells within lymphatic channels [14][15][16][17][18][19].…”
