Lutetium-177 Radiopharmeceutical Therapy Extravasation Lessons Learned

Abstract: Medical radiation safety officers (RSOs) need to understand the risk of extravasations associated with radiopharmaceutical infusion therapies, and this case study can help RSOs be prepared to respond to an extravasation at their facility.

“…For three common radiopharmaceutical isotopes, we determined appropriate values for volume and initial radioactivity based on typical clinical administrations with assumed extravasations of 25% and 50%. Rates of biological clearance were based on values found in the literature (Osborne et al 2021; Berry and Kendrick 2022). Table 1 shows details of each extravasation scenario we investigated.…”

“…An example of additional actions on how to evaluate the dose to the injection site has been published elsewhere for Lu-177 DOTATATE. 27 For Task (4c.iii. ), post-therapy procedures include determination of the administered activity, patient monitoring, patient release and radiation safety instruction, handling of certain situations after release, contamination checks, and area release.…”

“…We understand that not all procedures go as planned. But when they don't, we expect and need to be told, since these procedures directly or indirectly affect our care (1)(2)(3)(4)(5)(6)(7)(8). If we need additional therapy delivered or if we need to repeat an imaging procedure because of an extravasation, we expect this information to be shared with us and that actions be taken to prevent a repeat extravasation.…”
