Lymph node metastatic patterns and the development of multidisciplinary treatment for esophageal cancer

Abstract: Summary Abundant lymphatic flow and the anatomical location of the esophagus can result in the widespread distribution of lymph node metastasis of esophageal cancer from the cervical to the abdominal field. Historically, the Japan Esophageal Society and American Joint Committee on Cancer offer two different classifications of lymph node group location surrounding the esophagus. The location of sentinel lymph nodes in midthoracic esophageal cancer reflects the variety of lymphatic drainage routes… Show more

“…Pretherapeutic LNM may indicate that tumor has metastasized to distant LNs out of range during surgical dissection or has hematogenously spread to other organs. Esophageal cancer metastasizes to the LNs and other organs even at a relatively early stage 30,31 . Hence, if there is no pathological LNM in the site of dissection after multimodal therapy, recurrence should also be considered in other occult LNs outside the area of dissection or in distant organs in patients with pretherapeutic LNM.…”

“…Surgery entailed a transthoracic oesophagectomy with right thoracotomy and gastric tube reconstruction via the posterior mediastinal or retrosternal route and a two- or three-field lymph node dissection 22 , 23 . This has been the standard curative surgical procedure since before 2010 in Japan.…”

“…The five-year survival rate for CA currently stands at 10%–20%, significantly reduced for patients who do not undergo surgical resection, plummeting to 3%–5% ( 3 ). Indeed, the majority of CA cases present as advanced diseases, making neoadjuvant chemo/chemoradiotherapy the primary treatment option ( 4 ).…”
