Lymph node retrieval for colorectal cancer: Estimation of the minimum resection length to achieve at least 12 lymph nodes for the pathological analysis

Abstract: Abdominoperineal and Hartmann's resections are, in our series, high-risk operations that frequently do not produce the minimum number of lymph nodes necessary. These operations may require additional maneuvers such as mobilization of the splenic flexure to achieve the minimum length of bowel to resect.

“…Regarding surgical treatment we could observe that the right hemicolectomy was the most common operation 31.7% in this study. This was consistent with Tonini et al 16 who reported the highest incidence for right hemicolectomy 39% and Gravante et al 17 with 39.7%. Although the left colon was the higher incidence site, this could be explained by the fact that the left colon cancer has a variety of procedures, include left hemicolectomy, sigmoidectomy or even anterior resection.…”

“…According to tumor location, the most common site in this study is ano-rectal tumors which accounted for 30.3%, This figure was exactly like Gravante et al 17 who reported (30.4%) of rectal tumors of total colo-rectal cancer cases. The left colon, including tumor located from the splenic flexure down just before the recto-sigmoid junction, represented about 29.6% of the total cases.…”

“…17 There are many factors that can influence LN yield such as age, surgery, systemic diseases, and preoperative treatment. 18,19 Especially preoperative treatment in the form of (chemo)radiotherapy can lead to a reduced LN yield potentially leading to understaging. 18,20 High-quality pathological assessment of a TME specimen is therefore crucial.…”

“…[48][49][50] Thereby, it would be regarded to have high risk factor if the number of LNs harvested per specimen were fewer than 12, according to the guideline of National Comprehensive Cancer Network (NCCN). 14,51,52 Other studies also claimed that more LNs are needed for staging, decision making, and prognosis predicting. 53,54 Besides, the proportion of the number of involved LNs upon the examined LNs-defined as metastatic LN ratio (mLNR)has also been considered as a prognostic parameter in CRC, and high mLNR was significantly correlated with worse survival.…”
