Lymphadenectomy in colorectal cancer: does it make a difference?

Abstract

Zusammenfassung. Grundlagen: Die Frage der adäqua-ten Lymphadenektomie wird beim kolorektalen Karzinom seit mehreren Generationen ohne abschließendes Ergebnis diskutiert.Methodik: Ü bersichtsarbeit auf der Grundlage einer selektiven Literaturausarbeitung.Ergebnisse: Unsere Leitlinien empfehlen die radikale En-bloc-Resektion des tumortragenden Darmabschnitts mit zentraler Ligation der zugehörigen Gefäße, obwohl evidenz-basierte Studien fehlen. Die Resektion beinhaltet die systematische Lymphadenektomie des Lymphabflussgebiets entlang der zuführenden Blutgefäße und die NoTouch Technik. Die Lymphknotendissektion bestimmt das Ausmaß der Darmresektion, welche in Abhängigkeit der Tumorinfiltrationstiefe (pT) 5-10 cm proximalen und distalen Resektionsabstand beinhalten sollte. Der Lymphknotenstatus (pN) repräsentiert einen der wichtigsten Prognosefaktoren. Die Zahl resezierter Lymphknoten sowie des Lymphknotenquotienten ermöglicht ein noch detaillierteres Tumorstaging und eine präzisere Abschät-zung der Prognose. Die Angabe der Gesamtzahl resezierter Lymphknoten kann durch den Chirurgen, den Patienten sowie den Pathologen erheblich beeinflusst werden.Schlussfolgerungen: Neue Studien zeigen, dass nur eine kleine Gruppe der Patienten einen Prognosevorteil durch die radikale Lymphadenektomie beim kolorektalen Karzinom besitzen. Da wir zum gegenwärtigen Zeitpunkt diese Untergruppe prä-oder intraoperativ nicht identifizieren können, sollte weiterhin eine radikale Lymphadenektomie durchgeführt werden.Schlüsselwörter: Lymphadenektomie, Lymphknotenmetastasen, Lymphknotenquotienten, kolorektales Karzinom. Background:The question of what constitutes adequate lymphadenectomy in colorectal cancer has been discussed for several generations without a definitive conclusion being reached.Methods: Review of the literature. Results: Our guidelines recommend the radical enbloc resection of the tumor-bearing bowel with central ligation of its vessels, although evidence supporting this is limited. The resection includes a systematic lymphadenectomy of the lymphatic drainage along its arterial blood supply, and should be performed with a no-touch technique. The lymphatic dissection determines the extent of bowel resection, which should include -depending on the tumor infiltration depth (pT) -a 5-10 cm resection margin proximally and distally. The lymph node status (pN) is one of the most important prognostic factors. The calculation of the total number of resected lymph nodes and the lymph node ratio allows greater accuracy in tumor staging and a more precise estimation of prognosis. The total number of lymph nodes can be influenced by the surgeon, the patient, and the pathologist.Conclusions: New studies exist, which show that only small subgroups of patients receive a prognostic benefit from radical lymphadenectomy. Because we cannot yet identify these patients pre-or intraoperatively, we continue to perform radical lymphadenectomy.
