Lymphoma Involving the Heart: A Case Report

Abstract: Background: We describe a rare case of cardiac malignancy that counts for <1% of the primary cardiac tumors with a poor prognosis. Case summary: It's a rare case of a primary cardiac lymphoma in a 59-year-old patient who presented for recurrent pleuropericarditis with low-grade fever and night sweats. Investigations revealed an infiltrating mass in the right atrioventricular groove involving the right atrium and the right ventricular free wall. Pathology confirmed the diagnosis of non-Hodgkin's lymphoma. Chemo… Show more

“…Our patient presented with dyspnoea, reduced effort tolerance and palpitation. He had paroxysmal atrial flutter, signifying a potential involvement of the conduction system, especially with the tumour being in atrioventricular groove and in close proximity to the conduction pathway [ 9 , 10 ]. He also presented in tamponade necessitating an emergency pericardiocentesis.…”

“…T-LBL is a rare malignant lymphoma ( 4 ), accounting for about 2% of all lymphoma ( 3 , 4 ). The many complications associated with this tumor are often life-threatening, including cardiac arrhythmias, cardiac, tracheal and esophageal compression, and even death from cardiogenic shock and refractory heart failure ( 3 , 4 , 6 - 8 ). In this case, the patient was diagnosed with lymphoma, and presented with severe dyspnea on admission.…”

“…Precursor T-cell lymphoblastic lymphoma (T-LBL) is a rare type of malignant lymphoma ( 2 - 5 ), which has the characteristics of high invasion and proliferation with a very high degree of malignancy ( 4 , 5 ). In the early stage, the tumor will proliferate rapidly and invade extensively, compressing the heart, aorta, trachea, esophagus, and other organs ( 4 , 5 ), resulting in shock, acute dyspnea, and a series of emergencies, threatening the life of patients, and representing a difficult problem in clinical practice ( 1 , 3 , 5 - 8 ). Due to the wide variety of mediastinal tumors, especially lymphomas, accurate classification of tumors is the key to individualized precision therapy ( 1 , 9 ).…”

“…MRI is useful for detecting infiltrates of the myocardium and pericardium [1 , 14 , 16 , 17] . Lymphoma often presents as hyperintense on T2-weighted imaging and as hypointense on T1-weighted imaging, but it can also appear with heterogeneous signal intensity [15 , 17] .…”
