Macular Hole following Bartonella Henselae Neuroretinitis

Abstract: Cat scratch disease is a rare pathology and is most often considered benign. Serious complications can nonetheless occur, such as neuroretinitis, choroidal nodules, and disciform keratitis. The authors report a case of sequellar macular hole. They found only one previous report of macular hole caused by B henselae, which, contrary to their case, appeared rapidly 12 days after presentation.

“…8 It has been reported that cat scratch disease caused a macular hole in a 10-year-old girl 12 days after the diagnosis of neuroretinitis with posterior vitreous detachment and in an 11-year-old boy 7 months after the diagnosis of cat scratch disease. 9 Repeat optical coherence tomography in our patient at 29 days follow-up showed no evidence of a macular hole. B. henselae causes a self-limited infection and most patients recover without treatment.…”

“…Albini et al (1) noted vitritis, anterior uveitis, keratic precipitates, serous retinal detachment, and deep white patchy chorioretinal lesions in a 10-year-old girl with Bartonella neuroretinitis who later developed a full-thickness macular hole. Donnio et al (2) reported a case of treated B. henselae neuroretinitis in an 11-year-old boy with the formation of a macular hole 7 months after the infection. Other reported inflammatory causes of macular holes include idiopathic posterior uveitis, presumed ocular histoplasmosis, fungal endophthalmitis, intraocular nasal T/NK lymphoma, immune recovery uveitis, and juvenile rheumatoid arthritis after cataract extraction (1).…”
