Background: Anti-vascular endothelial growth factors are commonly used for treatment for central retinal vein occlusion. There have been numerous reports regarding both the benefits and disadvantages of its use. Recently, a rather surprising complication- development of macular holes have started emerging. Objectives: The purpose of this study was to evaluate if the patient's profile, baseline optical coherence tomography parameters, status of the vitreous and subsequent response pose a risk for macular hole formation in cases of central retinal vein occlusion central retinal vein occlusion following treatment with bevacizumab. Methodology: This prospective exploratory analytical study was conducted at a tertiary eye care center of Nepal and evaluated 97 eyes with central retinal vein occlusion undergoing treatment with bevacizumab. Results: Full Thickness Macular Hole developed in eight (8.24%) of the central retinal vein occlusion eyes treated with bevacizumab. Elderly age and eyes with cystoid edema at baseline were found to have greater risk of developing macular hole following treatment (OR: 8.97 , p<0.05). Twelve eyes (12.3%) of the eyes treated developed vitreoretinal interface anomalies and were the result of dramatic decrease in central macular thickness . All of them had cystoid macular edema at baseline. Five of them (42%) developed full thickness macular hole. Conclusion: Anti-vascular endothelial growth factor agents have been widely accepted as the first line of choice for treatment of macular edema secondary to central retinal vein occlusion. Cystoid macular edema, eyes of the elderly and those showing dramatic decrease in central macular thickness with the development of anomalous vitreoretinal interface are potential risk factors for full thickness macular hole.

PurposeThe formation of macular hole after receiving anti-vascular endothelial growth factor (anti-VEGF) therapy is rare. We report a case of macular hole that occurred after intravitreal injection of an anti-VEGF agent for age-related macular degeneration (AMD) in a patient, who underwent vitrectomy combined with choroidal neovascularization (CNV) removal.ObservationsA 64-year-old female with AMD affecting her right eye received an intravitreal injection of an anti-VEGF agent. After treatment, we identified a full thickness macular hole (MH) that was associated with the rapid resolution of the macular edema and contraction of the CNV. After performing vitrectomy combined with CNV removal, the MH closed and her visual acuity improved. Examination of the removed CNV revealed a network of microvessels devoid of pericytes.Conclusions and importanceThe present findings suggest that rapid resolution of macular edema and contraction of the CNV and/or mild increase in the vitreous traction after anti-VEGF therapy could potentially cause MH. CNV removal via the MH may be an acceptable procedure, if the MH remains open, the CNV is of the classic type, and it spares a central portion of the fovea.

Background: Color vision tests are routinely performed and are mandatory in most part of the world. However, in Nepal and many other developing countries, color vision may often be overlooked. We evaluated a possible burden of color vision in a group of patients who were specifically evaluated for a color vision defects. This study evalutes the awareness of color vision defect among the patients evaluated and highlights the importance of the color vision evaluation. Methods: A sequential group of 73 people from August to September 2017 specifically evaluated for color vision defect for recruitment of government employment were evaluated. Ishiharapseudo-isochromatic plates and Farnsworth-Munsell Dichotomous D-15 test were used for screening. Mean and Standard deviation were used for descriptive analysis of the data. Results: Fifty-seven were male and sixteen were female. The mean age was 23 years (SD ± 3.7). On evaluation of the color vision defect, 9 (12.3%) were found to have total color vision defect (achromatopsia), 3 (4%)-red-green defect and 1(1%) with blue red defect. None of the patients had undergone color vision test at eye hospital previously. There were 4 patients who were registered drivers who had color vision defect. Conclusions: Color vision is an important part of the vision. It should not be ignored.All of the patients evaluated were found to be unware of their condition. Early detection of color vision defects in individuals, if possible, at school level can help them to determine their careers and future endeavors at early stage.
