MAFLD: Renovation of clinical practice and disease awareness of fatty liver

Abstract: Recently, international expert panels have proposed a new definition of fatty liver: metabolic dysfunction‐associated fatty liver disease (MAFLD). MAFLD is not just a simple renaming of non‐alcoholic fatty liver disease (NAFLD). The unique feature of MAFLD is the inclusion of metabolic dysfunctions, which are high‐risk factors for events. In addition, MAFLD is independent of alcohol intake and the co‐existing causes of liver disease. This new concept of MAFLD may have a widespread impact on patients, medical d… Show more

“…Another important distinction is that MAFLD is independent of alcoholic intake or other liver diseases, including viral hepatitis. Therefore, patients with dual (or more) etiologies of liver disease can be diagnosed with “MAFLD and alcoholic liver disease” or “MAFLD and chronic hepatitis B (CH-B).” [ 49 , 50 ] Therefore, MAFLD allows us to investigate the impact of coexisting fatty liver in patients with viral hepatitis. Such assessments will also impact studies of viral hepatitis and MAFLD in their contribution to HCC development.…”

“…The negative effect of metabolic syndrome and insulin resistance on liver disease has been widely explained, and it is represented by a spectrum of conditions ranging from liver steatosis to NASH, and, in a later stage, liver fibrosis and cirrhosis [47][48][49]. No data are available on the risk of liver disease in CCS.…”

“…[10] This failure is likely explained at least partially by the underlying heterogeneity of MAFLD and their main disease drivers that failed to be captured under the NAFLD definition, making a one-sizefits-all approach unlikely. [11][12][13][14] The details of the pipeline of the investigational candidate drugs for MAFLD and potential reasons for the suboptimal performance of these drugs in clinical trials have been the subject of multiple recent reviews. [5,[15][16][17] Transformational changes and novel targets and concepts are needed to address MAFLD-related morbidity and mortality.…”
