Magnetic instrumentation and other applications of magnets in NOTES.

Abstract: Modern surgery is developing towards a minimally invasive approach. To minimize the trauma the number of ports is either limited as in single incision laparoscopic procedures or ports are introduced through natural orifices as in natural orifice translumenal endoscopic surgery (NOTES). To provide surgeons with appropriate instrumentation novel technologies are employed involving magnets. This article summarizes the theoretical background, technology and currently developed magnetic instrumentation for NOTES, l… Show more

“…First, permanent magnets, especially the Neodymiun type, have low resistance to corrosion inside the human body. Thus, an inert non-ferromagnetic coating (gold or epoxy resin) must be used to prevent release of harmful substances 17 . Second, the distance between magnets is a noteworthy factor in establishment of suitable counter-traction power.…”

“…With all the breaking news on NOTES published by the media, one might get an impression that NOTES is a valid option for patients today. Despite rapid progress in peritoneal cavity access, navigation skills, access side closure techniques and others, NOTES might only be a reality of tomorrow [1315], and then only for selected procedures. But even for those the implementation of NOTES will be to a large extent a patient-driven process.…”

“…Thus, all neodymium magnets should be isolated from contact with tissues and fluids inside the body. The shielding material must be inert to the human body and non-obstructive to the magnetic field ( e.g ., gold or epoxy)[ 29 ].…”

“…Unlike the clip-with-line, percutaneous traction, and sinker-assisted methods, MAG-ESD provides dynamic traction in a sense that the external magnet can be moved to change the direction of retraction. Permanent and electromagnets are the only two types of magnets available[ 29 ].…”
