Magnetic Resonance Imaging and Histopathology in a Case of Pleomorphic Adenoma of a Minor Salivary Gland in the Nasal Cavity

Abstract: Patient: Male, 56 Final Diagnosis: Pleomorphic adenoma Symptoms: Epistaxis Medication: — Clinical Procedure: — Specialty: Radiology Objective: Rare disease Background: Pleomorphic adenoma of the salivary gland is a common benign tumor of the parotid gland. However, pleomorphic adenoma arising in the minor salivary glands, including the nasopharynx, is uncommon. This report is o… Show more

“…1) FNAC was performed using a 23-gauge needle over swelling that showed similar cytomorphology. Smear showed sheets cuboidal polygonal epithelial cell, at places entrapped and surrounded by fibro myxoid material [3]. All these findings were suggestive of pleomorphic adenoma, a case arising from the submandibular gland extending into the neck causing airway obstruction.…”

“…CT may show an enhancing expansile lesion, and MRI typically demonstrates a well‐defined lesion with low signal intensity on T1 MRI and variable signal intensity on T2 with heterogeneous contrast enhancement 994,996 . Preoperative tissue sampling is critical for treatment planning, particularly in tumors with aggressive behavior (skull base extension, bony invasion, PNI) or high‐risk imaging features (large size, poorly defined margins, T2 hypointensity) that suggest malignant potential 992 .…”

“…Nasal cavity pleomorphic adenomas are rare benign mixed tumors of the salivary glands. Making up 0.4 % of all pleomorphic adenomas [ 1 ], they mostly arise in the nasal septal mucosa with a reported incidence of 82.5–90 % [ 2 ], and have predominant myoepithelial cellularity with reduced stromal component compared to pleomorphic adenomas in other sites. We present a rare case of pleomorphic adenoma arising as a septal mass causing symptoms of unilateral nasal obstruction, successfully managed with endoscopic endonasal resection.…”
