Major Liver Resection for Hilar Cholangiocarcinoma

Abstract: Between 1968 and 1984 liver resection with curative attempt was performed in 22 patients with hilar cholangiocarcinoma. Right lobectomy was performed in 4 patients, extended right lobectomy in 7, left lobectomy in 8, and excision of the median segment segment of the left lobe (segment IV) in 3. Bilio-enteric continuity was restored by hepatocholedochostomy in 17 patients and hepatojejunostomy in 4. (One patient had external transhepatic catheter drainage and no internal bile drainage.) Operative mortality rate… Show more

“…Excision of the caudate lobe was done in 20 of the 28 (71 %) patients (10,(22)(23)(24). Systematic lymph node dissection was done in 24 (86%) patients (10,12,26,27). There were nine vascular reconstructions, including: four segmental excision of main portal vein; one primary reconstruction of right portal vein; two segmental excisions of right hepatic artery; one primary reconstruction of the artery of the left lateral segment; and one replacement of both the main portal vein and proper hepatic artery graft with cadaver iliac vein and artery grafts, respectively (28)(29)(30)(31)(32)(33).…”

“…However, the operative mortality is high and the 5-year survival rate remains disappointing (7). Local tumor recurrence is the major cause of death for patients who undergo resection (8).…”
