Making prospective entrustment decisions: Knowing limits, seeking help and defaulting

Abstract: Introduction Many studies focused on entrustment have not considered prospective entrustment decisions, where clinical competency and entrustment committees determine how much supervision trainees will require in the future for tasks occurring in not completely known contexts. The authors sought to explore factors that influence and determine prospective entrustment decisions made by members of such committees in graduate medical education (GME) and undergraduate medical education (UME). Methods The authors co… Show more

“…Our findings highlight substantial challenges in making prospective determinations about readiness of graduating medical students for indirect supervision in the Core EPAs framework at the scale of entire medical school classes. 7,8,33,45 Results also suggest important gaps in readiness for a subset of Core EPAs (ie, EPAs 4, 8, 10, 11, and 13). 45 Among next steps suggested by these results are improvement in curriculum and assessment specific to these EPAs at participating schools; further improvements in systems for programmatic assessment and data curation; and consideration of revision of what are considered Core EPAs on a specialty-specific basis for incoming interns.…”

“…In the article, Making Prospective Entrustment Decisions: Knowing Limits, Seeking Help, and Defaulting , Schumacher et al 3 paint a concerning picture of clinical competency committees (CCCs) which have inadequate data and default to progress trainees (prospective entrustment) in the absence of red flags. The authors effectively highlight the resultant threat to patient safety in the graduation of trainees who may not have the requisite competencies to prospectively entrust independent practice.…”
