Malabsorption of vitamin B12 and intrinsic factor secretion during biguanide therapy

Abstract: Summary. In a survey of 46 randomly selected diabetic patients on biguanide therapy, 30% had malabsorption of vitamin B12. Withdrawal of the drug resulted in normal absorption in only half of those with malabsorption. In most patients with persistent malabsorption, the results of absorption tests with exogenous intrinsic factor suggested the diagnosis of coincidental intrinsic factor deficiency. Further considerations, however, led to the concept that biguanides can induce malabsorption by two different mechan… Show more

“…There is an age-related decline in serum cobalamin (S-B 12 ) concentrations (Nilsson-Ehle et al, 1991), and cobalamin (vitamin B 12 ) deficiency is more common in the elderly (Nilsson-Ehle, 1998). This might be due to an increased prevalence of atrophic gastritis (Streeter et al, 1976;Ihamäki et al, 1979;Krasinski et al, 1986;Carmel et al, 1987;Jones et al, 1987) and certain medicaments (Adams et al, 1983;Belaiche et al, 1983;Kittang & Schjönsby, 1987;Lindstedt, 1999). The extent of vitamin B 12 malnutrition in the elderly is unknown but concern relates to the observation that microwave heating of food can cause degradation of hydroxo-cobalamin (Watanabe et al, 1998).…”

“…Vitamin B 12 deficiency was very significant in high-risk populations like older individuals, malabsorptive states like in gastric and ileal resection or autoimmune gastric mucosal disease or gastric atrophy, coeliac disease, Crohn's disease, ileitis, pancreatic insufficiency, vegetarians and long-term use of proton pump inhibitors, histamine (H 2 ) receptor antagonists or biguanides. [3][4][5] In pernicious anaemia, the gastric parietal cells are targeted by the autoimmune process. Hence, the production of intrinsic factor is affected leading to defective absorption of vitamin B 12.…”

“…Так при хроническом приеме метформина часто возникают желудочно-кишечные расстройства (диарея), нару-шения всасывания витаминов группы B и фолатов [35]. Дефицит этих факторов ведет к увеличению в плаз-ме уровня гомоцистеина, который ускоряет прогрес-сирование сосудистых поражений за счет неблаго-приятного воздействия на тромбоциты, факторы свер-тывания крови и эндотелий сосудов [36].…”
