Purpose of reviewThis article aims to present the current picture on conceptualization, diagnosis and care provision of personality disorders (PD) in modern Russia. These issues are highly relevant to Russian psychiatry, which has a rich historical tradition of analysing psychopathic constitution, personality types and dynamics. Recent findingsThis narrative review covers the main tendencies in the recent Russian academic literature devoted to PD. PD are most frequently investigated in the frames of forensic psychiatry, in which expert conclusions and assessment of the risk factors for socially dangerous behaviour are particularly important. Another area of research concerns comorbidity, since PD often affect other health conditions or are co-occurrent with other mental or behavioural problems. Specialists have also focused on the challenges of therapy in PD. SummaryReviewed manuscripts show a pathogenic and pathoplastic role of personality factors in manifestation of other mental, behavioural or somatic disorders. Analysis of pathocharacterological profile and selfregulation processes is a major component in assessing and diagnosing PD. The principal perspectives in this particular area focus on evidence-based research of mechanisms of personality pathology and validation of targeted personalized combined therapeutic approaches.

<p style="text-align: justify;">The issue of suicidal behavior in adolescents is one of the most disturbing and complex. The present work is aimed to draw attention to the problem of suicidal behavior in adolescents in modern Russian reality. While analyzing cases of severe injuries due to suicidal behavior in adolescents who were admitted to the Clinical and Research Institute of Emergency Pediatric Surgery and Trauma (CRIEPST) in Moscow (Russia) , the authors stated their significant increase in recent years . In 2016-2023, 58 children after suicidal attempts were hospitalized to CRIEPST. All children were teenagers (11-17 years old). The authors not only noted an increased risk of suicide in 14-16 age group of teenagers, but they also noted gender features in this group. Two groups of suicide causation were defined as well: psychopathological processes and affective, impulsive reactions. After summarizing findings obtained in CRIEPST , the researchers have stated an increase in the number of psychopathological predispositions in children in the recent years. An increased number of cases with suicidal behavior in adolescents and predominance of psychopathological processes as risk factors should make adults be more attentive to tendencies in suicide causation and their prevention. The data presented in the article are preliminary and incomplete, as they do not cover cases that did not lead to severe somatic consequences.</p>

Rumination as a psychological mechanism in the suicidal behavior formation and maintenance

The role of rumination in formation and maintenance of suicidal thoughts (ST) and suicidal behavior (SB) is specified in the article. In leading foreign models SB is considered to be the process of phase restructuring and the resultant intersection of factors (Joiner's interpersonal theory, O'Connor's motivational-volitional model, Klonsky's three-step model). Analysis of traditional SB models shows insufficient consideration of metacognitive system-dynamic phenomena, which may explain the limitations of predictive theories, especially in relation to adolescents. Ruminations as one of such phenomena are designated as a component of the process of repetitive negative thinking, which intensifies the experience of problems insolubility and sets the basis for comprehension of suicide as a potential way to “complete” the situation. The typologies of ruminations and the contribution of each subtype to the maintenance of ST, SB are considered: 1) ruminations as a trait and a state; 2) brooding ruminations and reflection; 3) pre- and post-event ruminations. The interrelation of rumination and insomnia in the form of interdependent cycle of accumulation of undischarged psychophysiological tension in the context of SB formation is demonstrated. Russian psychology employs the methodology of the cultural and activity approach to explain the formation of SB, and analyzes the motives of personality and the possibility of their implementation, taking into account the systems of mediation and regulation of mental activity and the laws of ontogenesis. The role of metacognitive dysregulation of social anxiety in the formation of SB in adolescence is shown. The significance of the dynamic mechanism of SB provoking accumulation of tension outside the situation and contributing to self-reinforcement of symptoms is underlined.
