Maldynia: Pathophysiology and Management of Neuropathic and Maladaptive Pain—A Report of the AMA Council on Science and Public Health

Abstract: As defined, maldynia is a multidimensional process that may warrant consideration as a chronic disease not only affecting sensory and emotional processing but also producing an altered brain state based on both functional imaging and macroscopic measurements. However, the absolute clinical value of this definition is not established.

“…This is followed by a process called deafferentation and the proximal portion of the severed nerve sprouts to form neuromas [18]. There is an increased accumulation of molecules enhancing the expression of sodium channels in these neuromas that results in hype-excitability and spontaneous discharges [19]. This abnormal peripheral activity is thought to be a potential source of the stump pain, including phantom pain [18].…”

“…Some neurons in the areas of spinal cord that are not responsible for pain transmission also sprout into the Lamina II of the dorsal horn of the spinal cord which is the area involved in the transmission of nociceptive afferent inputs [18, 19]. This is followed by increased neuronal activity, expansion of the neuronal receptive field, and hyperexcitability of other regions.…”

“…Moreover, it is often stated that chronic pain is pain that extends beyond the expected period of healing, and that the pain is without purpose and a disease in its own right [173]. In this context, some authors use concepts like pathological pain, or maladaptive pain, or maldynia (Greek for "bad pain") [71,96]. It is sometimes stated that the vital importance of acute pain might be an important background to why pain can become chronic.…”

“…In addition, there is a growing consensus that multiple chemical sensitivity, post traumatic stress disorder, depression, primary dysmenorrhea, irritable bowel syndrome, periodic limb movement disorder and restless leg syndrome are strong candidates for inclusion as central sensitization syndromes. “…The pathways are few, and have fairly specific therapeutic protocols associated with them. [2,5,7,8,[13][14][15]17,19,20,32] There are two distinctly different approaches to CSS therapy. The first, and historically more common, is to address the effects of CS after it has occurred.…”
