Male Accessory Gland Infection: Relevance of Serum Total Testosterone Levels

Abstract: Aim of the present study was to evaluate the different ultrasound characterization of fertile symptomatic patients with MAGI (male accessory gland infection) according to different serum concentrations of total T (TT). We analyzed the ultrasound and hormonal data of 200 patients aged between 24.0 and 67.0 years. Patients were divided into six groups according to the sextile distribution of TT. Patients with serum concentrations of TT < 3.6 ng mL−1 had a higher mean duration of symptoms compared to the other ex… Show more

“…We recently reported that patients with low concentrations of total testosterone (TT) have a higher frequency of male accessory gland inflammation (MAGI) and that they have a greater extension of the ultrasonographic criteria suggestive of complicated forms: prostato‐vesciculitis and prostato‐vesiculo‐epididymitis, conditions associated with lower quality of sperm parameters and more severe symptoms than prostatitis (P) that represents the uncomplicated form of MAGI (Condorelli et al., ; La Vignera, Vicari, Condorelli, D'Agata, & Calogero, ). It well known that the leucocytospermia (concentrations of peroxidase‐positive leucocytes in the ejaculate >1 million/ml) represents an indication for further microbiological evaluation but is not a sufficient criterion for the diagnosis of MAGI (Comhaire, Verschraegen, & Vermeulen, ; WHO, ) (Table ).…”

“…Several studies have shown that the ultrasound examination of the prostato‐vesicular tract and epididymis is an important diagnostic tool. Indeed, a comprehensive review on ultrasound physiopathology of male seminal tract confirmed the importance of ultrasonographic evaluation in the diagnosis of male genital tract inflammatory diseases (Condorelli et al., ).…”

“…Detailed scrotal and prostato‐vesicular ultrasonographic examinations were also performed to investigate the clinical presence of MAGI. The diagnosis of MAGI and ultrasonographic evaluations were carried out according to criteria previously published and summarised in Table (Condorelli et al., ; La Vignera et al., ). MAGI diagnoses were also subcategorised as MAGI‐Prostatitis (MAGI‐P), MAGI‐Prostatitis‐Vesiculitis (MAGI‐PV) or MAGI‐Prostatitis‐Vesiculitis‐Epididymitis (MAGI‐PVE) according to the same criteria.…”

“…Moreover with US evaluation was reported a reduced frequency of bilateral PVE in patients with normal serum concentrations of TT [26], suggesting a protective role of androgens on the greater extension of the inflammatory process. At the same time, the eugonadal patients had a reduced frequency of fibro-sclerotic variant of MAGI, that represent an US form associated with persistent low quality of sperm parameters after pharmacological treatment ( Figure 1) [26,27]. [27]; On the left dilated seminal vesicle with increased anteroposterior diameter (threshold: 14 mm) and polycyclic intraluminal areas.…”
