Malignant gastric outlet obstruction managed by endoscopic stenting: a prospective single-centre study

Abstract: Palliative stenting for advanced malignant upper GI-tract tumours at a tertiary Hepato-Pancreato-Biliary Unit is a safe, feasible and effective alternative to surgical bypass with a short hospital stay and prompt improvement of food intake.

“…There is no literature on this particular stent in the treatment of benign stricturing, although its use has been documented in malignant gastric outlet obstruction. 8 The management of patients with benign duodenal stenosis is problematic, and for those with resistant strictures, laparoscopic gastroenterostomy remains the treatment of choice in those fit for surgery. Because the initial covered stent fractured after 2 months, for the use of a covered SEMS in this scenario there would need to be a more durable covered stent developed to manage the longer duration of benign disease.…”

“…Many previous studies concerning GOO and SEMS have included patients with obstruction in the surgical anastomosis or afferent/efferent limb [3][4][5][6][7]. For this study, we wanted to exclude the patients with gastrojejunal or jejunal strictures and curable malignancies such as intraabdominal lymphoma to compose a uniform entity of malignancies causing GOO.…”

“…Endoscopic stenting appears to be a safe and effective treatment for this disorder [3][4][5][6][7]. Enteral stents usually are inserted through an endoscope, but stent placement with fluoroscopic guidance alone is another option [8][9][10].…”

“…Furthermore, a rate of conversion to open surgery of 20% has been reported. Endoscopic SEMS insertion appears to be superior to both methods with its technical success rate above 90% (6). It also has the advantages of enabling oral intake within a shorter time and shortening the hospital stay.…”
