Malignant Hyperthermia

Abstract: Malignant hyperthermia (MH) is a medical emergency that all perioperative nurses should be prepared to handle. Patients with the inherited MH trait have a rare skeletal muscle disease that causes them to develop life-threatening hyperthermia (ie, body temperatures of 43.3 degrees C [110.0 degrees F] or higher) at the time MH-triggering agents are administered to induce general anesthesia or shortly thereafter. The incidence of MH episodes is reported to be 1 in every 12,000 pediatric anesthetic procedures and … Show more

“…This adverse effect of fever is commonly associated with a variety of factors including tricyclic antidepressant poisoning,4 overdose of quinine,8 amphetamines or digitalis,2 long QT intervals,11, 12 long QT and/or acute rheumatic fever,5, 6 the Brugada syndrome,10 malignant hyperthermia induced by anesthesia 3. Fever-induced VF has been reported in humans without detectable structural abnormalities or other factors known to precipitate sudden cardiac death 9.…”

“…If the triggering agent is continued, other clinical signs of MH such as tachycardia, temperature elevation, and acidosis will manifest. An elevated end-tidal carbon dioxide is felt to be the most valuable early sign of malignant hyperthermia based on multiple studies (Byers & Green, 1992;Dunn, 1997;Kaplan, 1997;Rosenberg, 1996).…”

“…Continuous skeletal muscle rigidity, hypermetabolism, hypercapnea, tachypnea, and tachycardia that can result in cardiac arrest characterize the MH syndrome if left untreated (Dunn, 1997). According to MHAUS consultants, Karan and Greenberg (1997), recent trends from the MH Hotline indicate that more episodes are having a wider Malignant Hyperthermia 3 variation in clinical signs and presenting symptoms.…”
