Malignant pleural mesothelioma without asbestos exposure with distant metastasis in a peripheral lymph node: A case report

Abstract: SUMMARYMalignant mesothelioma is an uncommon pleural neoplasm and usually associated with inhalation exposure to asbestos. About 20% of the patients have no demonstrable exposure to asbestos. It rarely metastasizes in peripheral lymph nodes. Here is a case report of malignant pleural mesothelioma without asbestos exposure with cervical lymph node metastasis

“…Review studies documented that axillary lymph node metastasis is exceptionally rare even in cases with chest wall involvement. [ 1 5 6 ] It is exceptional for patients to present with peripheral lymphadenopathy prior to the detection of the primary tumor. [ 7 ] Till date according to the literature, 14 cases of malignant mesotheliomas involving pleura, pericardium, and peritoneum have been diagnosed initially by extrathoracic lymph node study prior to the detection of primary tumor.…”

“…India has not reported a single case of mesothelioma to the WHO mortality database 29 30. There are rare case reports of mesothelioma from India, wherein the authors concluded that they have no causal relation to asbestos 31–33. The probable reasons as to why asbestos-related mesotheliomas have not been reported from India, like other occupational and environmental diseases have been reviewed earlier 3 34 35.…”

“…Despite being clinically similar to asbestos‐related neoplasms, this tumor has a distinct pathological entity, and immunoprofile evaluation has confirmed its mesenchymal origin. The tumor arises from the pleura, is either visceral (80%) or parietal (20%) and may undergo malignant transformation . Thermoablation techniques such as cryoablation, radiofrequency ablation (RFA), microwave ablation (MWA), laser ablation, and high intensity focused ultrasound (HIFU), have an established role in the treatment of localized tumors, especially in cases of well differentiated and/or radio‐resistant neoplasms.…”
