Malignant Transformation of Clival Chordoma After Gamma Knife Surgery -Case Report-

Abstract: A 54-year-old woman presented a midline clival tumor manifesting as right abducens palsy in May 1997. Magnetic resonance (MR) imaging revealed a midline clival tumor. She underwent surgery twice with the transsphenoidal approach and gamma knife surgery for residual tumor. The histological diagnosis was chordoma. MR imaging revealed that the tumor had extended to the right cerebellopontine angle, with spinal seeding in February 2002. She underwent partial removal of the right cerebellopontine angle tumor. The h… Show more

“…An explanation for the mechanism of spinal seeding is detachment of tumor cells and their seeding to the lower parts of the spinal leptomeninges (drop metastases) such as the case in medulloblastomas. Judging from the intracranial MRI images of previous 2 , 9 , 11 , 14) and our cases, extension into the cerebellopontine region and encroachment into the brain stem of the chordoma may be important causes of the occurrence of spinal metastasis. Few reports indicate that the faster growth rate of chordoma is associated with a Ki-67 labeling index of more than 6%.…”

“…9 Metastasis of clival chordoma after gamma knife surgery has also been reported. 10 Dissemination of tumor cells by transgressing the dura mater could be one of the pathogenesis of CSF dissemination. 11 In the report by Shinde and Monipanda 12 (a case of disseminated tumor on postmortem examination without prior surgical intervention), this may have been the case.…”

“…CT shows lobulated, sharply restricted osteolysis. In T2-weighted sequences, they appear with a high intensity and high contrast enhancement [5] commonly appear in the petro-occipital fissure and clivus, and rare cases of chondrosarcoma are found in the temporomandibular joint [6] . Chondrosarcomas show the same T2-weighted images as chondromas.…”
