Mallampati class changes during pregnancy, labour, and after delivery: can these be predicted?

Abstract: The incidence of Mallampati classes 3 and 4 increases during labour compared with the pre-labour period, and these changes are not fully reversed by 48 h after delivery. This work confirms the absolute necessity of examining the airway before anaesthetic management in obstetric patients.

“…MMT is also influenced by the patient's ethnic group: tracheal intubation may be more difficult in Asian than Caucasian patients 14 . The incidence of modified Mallampati Classes 3 and 4 also increases during labor compared with the pre-labor period 15 , and these changes are not fully reversed within 48 hours after delivery 16 . Our study has several limitations that are primarily due to logistic conditions.…”

“…Diffi cult airway is one of the major concerns when general anesthesia was given for cesarean section because of the following two reasons: fi rst, pregnancy-induced fi broelastic edema results in a signifi cant reduction in the space of oral and cavum larynges 9 ; second, movement limitation of cervical spine results from pregnancy obesity 23 . These factors lead to an increase in the incidence of Mallampati classes 3 and 4 9 .…”

“…These factors lead to an increase in the incidence of Mallampati classes 3 and 4 9 . In addition to these factors, our patient had a severely limited mouth opening and displaced upper trachea that undoubtedly increases the diffi culty of airway management during general anesthesia.…”

“…However, the odds ratio for difficult intubation for Mallampati III or IV scores is 5.89 (95% confidence interval (CI), 4.74-7.32) 18 . The Mallampati classification has been shown to change during pregnancy and to advance by one or two classes due to changes in pregnancy 19 . It is obvious that none of the tests mentioned above is specific for prediction of difficult airway.…”
