Management of Complicated Hydatid Cyst of the Thorax

Abstract: Complicated hydatid cyst may have different clinical manifestations and may present radiologically as a primary lung tumour. In patients with suspicious lung masses owing to endemic area, history of a hydatid cyst or contralateral or extrathoracic hydatid cyst involvement at the same time should indicate a complicated pulmonary hydatid cyst. Preoperative anthelmintic therapy must be avoided owing to the risk of perforation. Treatment of a complicated hydatid cyst differs from that of an intact hydatid cyst. An… Show more

“…Atelectasis can be caused by compression of the HC and may facilitate the development of pneumonia. Bronchiectasis can be seen in the distal parenchyma of HCs due to bronchial obstruction and parenchyma destruction [17]. As another associated finding, pleural thickening or free fluid can develop without intrapleural rupture of the HC [18].…”

“…This may necessitate lung resection in the form of wide wedge resection or even lobectomy (83). Another indication for anatomic resection is a ruptured or complicated hydatid cyst that cannot be differentiated convincingly from lung cancer or aspergilloma (84). Some large cysts may also compel a lobectomy when lesser resection may preserve minimal healthy parenchyma.…”

“…Although the basic treatment for pulmonary hydatid cysts is surgery, the purpose of the chemotherapy regimen that we performed was to reduce the risk of recurrence associated with hydatid cysts. [13][14][15] Albendazole or mebendazole are common anthelmintics used in the medical treatment of hydatid cysts. Albendazole is normally the first choice of treatment since it has a high plasma level and is highly absorbed by the gastrointestinal system.…”
