Management of Hand Surgery Services during the Novel Coronavirus Disease 2019 Pandemic

Abstract: Introduction The novel coronavirus disease 2019 (COVID-19) pandemic is an international public health emergency. Health systems must plan for the increasing requirements for critical care during the COVID-19 pandemic. The aim of this article is to offer strategies for hand surgeons to planning and implementing their response during the COVID-19 pandemic and in the subsequent phases to preserve health care system functioning. Materials and Methods Assessment of the principles of management related to COVID-19 m… Show more

“…The function of the reconstructive surgeon is to attain a satisfactory aesthetic result. Reconstruction is discussed when probable, but thought is given to minimising operating times, in-hospital stay (day case and 24 hours stay procedures), risk of complications and number of out-patient visits (e.g., tissue expander inflation) during the pandemic to control the risk of SARS-CoV-2 exposure to the patient and health care staff [ 59 ], [ 60 ]. Only the breast with cancer is addressed to avoid prolonged surgery by avoiding concurrent contralateral balancing procedures.…”

“…A negative pressure isolation room is commonly used for patients with communicable diseases, especially airborne infections. Many facilities are choosing to place potentially infected COVID-19 patients in isolation rooms when resources allow (Al-Benna 2020c). In order to ensure the safety of other patients, staff and visitors, it is important that the isolation room contain negative air pressure.…”

“…ACE2 is a zinc metalloprotease which shares 42% amino acid homology with angiotensin-converting enzyme (ACE) in its catalytic domain, and consists of 805 amino acids including a C-terminal membrane binding domain and a 17-amino acid N-terminal signal sequence [10,11]. ACE2 contains one HEXXH zinc-binding motif and is able to hydrolyze angiotensin I (AT1) to produce angiotensin- (1)(2)(3)(4)(5)(6)(7)(8)(9) and also has a high affinity for angiotensin II (AT2) to generate angiotensin (1-7) [AT(1-7)] [7]. AT2, the major component of the ACE/AT2/AT1 (AT2 type 1) pathway, facilitates vasoconstriction, promotes cell proliferation and maintains fluid homeostasis [12].…”
