Management of maxillary alveolar bone fracture and severely intruded maxillary central incisor: report of a case

Abstract: An 11-year-old male who injured his maxilla and right maxillary central incisor and lip during a fall was presented to our hospital. His lower lip and upper gingiva were lacerated with swelling and epistaxis, and he had a maxillary alveolar bone fracture and severe intrusion of the right maxillary central incisor, which had penetrated the floor of the nasal cavity with avulsion. Under local anesthesia, we repositioned the incisor and bone segment and fixed them with a titanium micromesh plate and self-tapping … Show more

“…Segmental displacements of alveolar bone can be clinically observed 1,29 and additional imaging exams are needed to provide further information such as fracture location, extension and relationship with important anatomical structures. A few case reports described the diagnosis of alveolar fracture by using radiographs 3,10 while some other case reports affirmed that CBCT is an effective diagnostic method for minor alveolar fractures 11,15,18 .…”

“…6 Alveolar fractures occur concomitantly with lateral luxation, intrusion, avulsion or trauma resulting in alveolar displacement. 3,[32][33][34][35][36][37][38] Alveolar fracture diagnosis is fundamental because it may result in pulpal necrosis, tooth loss, external root resorption or alveolar bone loss. 4 According to Diangelis et al 39 and Patel et al, 40 after fracture detection, it is necessary to reposition and immobilize the bone fragments for 4 weeks, suture lacerated soft tissues, administer antibiotic therapy when necessary and provide clinical and radiographic follow-up.…”
