Management of Neglected Lateral Condyle Fracture of Humerus

Abstract: Background:Implant for fixation of neglected fracture lateral condyle humerus remains an issue of controversy. This study compares the clinical and radiological outcome of fixation with Kirschner wire (K-wire) and with cancellous screw (CS) in neglected fracture lateral condyle humerus.Materials and Methods:42 patients of neglected lateral condyle humerus fracture, treated either by open reduction and internal fixation (ORIF) with K-wire or ORIF with CS were included in study. The comparisons were made in term… Show more

“…In lateral condylar fractures with no shift or a displacement of less than 2 mm, long arm cast immobilization is effective [1]. However, due to the need to restore the integrity of the articular surface of the distal humerus, internal xation is considered the optimal choice for most displaced lateral condylar fractures [1][2][3][4][5][6][7][8][9]. Due to epiphysis, standard x-ray examination of the lateral elbow does not provide a full, clear view of metaphyseal fractures.…”

“…Due to epiphysis, standard x-ray examination of the lateral elbow does not provide a full, clear view of metaphyseal fractures. Therefore, clinicians are unable to properly assess the degree of fracture displacement, leading to possible neglect and misdiagnosis of fractures [2][3][4]. If a fracture is missed, the treatment becomes di cult and bony abnormalities can occur.…”

“…in 2018 [17] suggests that there was no difference in the treatment modalities. However, immobilization time was less in cancellous screw group which is consistent with our study.…”

“…There is also a wide variation about the choice of fixation-with k-wires and screw-wire combination being the common two methods used. Ranjan et al in 2018 were the first to compare these two methods of fixation for neglected fractures [29]. Though they found that the functional scores to be statistically similar in both the methods, the period of immobilization was significantly higher in k-wires as compared to CC screws.…”
