Management of Patients Coinfected With HCV and HIV: A Close Look at the Role for Direct-Acting Antivirals

Abstract: With the development of effective therapies against human immunodeficiency virus (HIV), hepatitis C virus (HCV) infection has become a major cause of morbidity and mortality among patients with both infections (coinfection). In addition to the high prevalence of chronic HCV, particularly among HIV-infected injection drug users, the rate of incident HIV infections is increasing among HIV-infected men who have sex with men, leading to recommendations for education and screening for HCV in this population. Liver … Show more

“…Fifteen to 30% of HIV-infected patients are also infected with HCV, and coinfected patients have a significantly higher rate of HCV disease progression, resulting in high mortality rates due to liver disease in this population (31). As such, any treatment for the cure of HCV infection would ideally be compatible with antiretroviral medications utilized for treatment of HIV, and in vitro combination studies were performed combining LDV with two common HIV inhibitors of various classes.…”

“…oinfection with hepatitis C virus (HCV) is an important cause of morbidity and mortality among persons infected with HIV (1,2). Disease progression is accelerated, and coinfected patients respond poorly to treatment with pegylated alpha interferon and ribavirin (Peg-IFN/RBV) compared to those infected only with HCV (monoinfection).…”

“…Finally, drug-drug interactions must be carefully considered, especially in direct-acting antivirals (DAAs such as HCV-protease and polymerase inhibitors) [73][74][75]. To this end, several studies have described drug-drug interactions between boceprevir, telaprevir and antiretrovirals in normal subjects.…”
