Management of pilonidal disease

Abstract: As with any surgical intervention, the ideal treatment for pilonidal disease would be simple and cost-effective, cause minimal pain, have a limited hospital stay, low recurrence rate and require minimal time off from school or work. Less invasive procedures for pilonidal disease may be favourable as an initial approach for these patients reserving complex surgical treatment for refractory disease.

“…Pilonidal disease and its treatment cause significant morbidity and broad impacts on quality of life in adolescents during an already fraught developmental period. Treatments have seen incremental improvement over the past century but still lack a standard algorithm and may not be routinely curative [11]. There are little published data on the cost of pilonidal care in the United States.…”

“…For adolescents, the activity restrictions mandated through the perioperative period and the painful, embarrassing side effects associated with recurrences often lead to a disruption of schooling and social life during an important time in development [3,10]. Minimally invasive approaches such as sinusectomy are associated with minimal social disruption and no activity restrictions [11][12][13][14][15]. Additionally, smaller studies have shown promising results in the application of laser epilation in the primary or secondary management of pilonidal disease [16][17][18][19].…”

“…For this, the complication rate of the procedure should be low, the hospital stay should be short and the recurrence rates should be low. 2,4,5 As a definite method cannot be established regarding aforementioned features, the V-Y flap and the Limberg flap, which are two popular methods, were compared in this study. 6,7 There are only three studies in the English literature comparing the V-Y flap and Limberg flap.…”

“…However, although many conservative and surgical methods have been defined, recurrence rates are still high and the search for the ideal treatment is ongoing. 2,3 Although the treatment of pilonidal sinus disease in the form of acute abscess is standard (drainage), the treatment of chronic and recurrent cases is not clear. Conservative treatment methods are recommended for simple cases with single orifice and short tracts.…”
