Management of Pre-eclampsia and Eclampsia: A Simulation

Abstract: IntroductionPre-eclampsia is a hypertensive disorder in pregnancy. Maternal sequelae that may occur include impaired liver function, disseminated intravascular coagulation, seizures (eclampsia), stroke, and death. Thus, providers should know how to recognize (diagnose) and treat pre-eclampsia and eclampsia.MethodsA simulator with noninvasive blood pressure monitoring was used. Transducers for fetal heart rate and contraction monitoring were placed on the simulator, which represented the patient. After obtainin… Show more

“…It is well established that in-person simulation increases provider readiness and knowledge ( 23 32 ). Additionally, studies have provided evidence that the benefits of in-person simulation courses are comparable among active in-person participants versus passive in-person observers ( 33 36 ).…”

“…Globally, 4-5% pregnant women are affected by PE, who face higher risks of seizures or falling into coma if the condition deteriorates into eclampsia, which threatens the life of both the mother and the baby ( 18 , 19 ). The past two decades have witnessed promising advances in terms of the accuracy and efficiency of PE diagnosis, in addition to improvements in the clinical management of this disease ( 20 , 21 ). However, the mechanism underlying the pathogenesis of PE remains to be fully elucidated ( 20 , 21 ).…”

However, the mechanism underlying the pathogenesis of PE remains to be fully elucidated ( 20 , 21 ). Trophoblasts can differentiate into highly proliferative and invasive extravillous trophoblasts with physiology similar to that of cancer cells, which can invade the endometrium and anchor the placenta to the uterus ( 22 ).…”

“…According to the national hospital discharge survey, the prevalence of hypertensive disorders in pregnancy (preeclampsia, eclampsia, gestational hypertension, and chronic hypertension) was estimated at 5.9% in the United States. The survey monitored about 39 million newborns over a 10-year period [ 15 , 16 ]. The study also showed that women with preeclampsia or eclampsia had a 3-25-fold increased risk of serious complications during their index pregnancies, including placental abruption, disseminated intravascular coagulation, pulmonary edema, and aspiration pneumonia [ 17 ].…”
