Management of the paralyzed face using temporalis tendon transfer via intraoral and transcutaneous approach

Abstract: Temporalis tendon transfer is a technique for dynamic facial reanimation. Since its inception, nearly 80 years ago, it has undergone a wealth of innovation to produce the modern operation. Temporalis tendon transfer is a relatively minimally invasive technique for the dynamic reanimation of the paralyzed face. This technique can produce significant and appropriate movement of the lateral oral commissure, more closely mimicking the normal side. The aim of this article is to review the technique of temporalis te… Show more

“…In selected cases, a smile procedure can be performed, either by nerve transfer technique, which uses the masseteric nerve to restore innervation in the free gracilis, or by muscle transfer technique (lengthening of the temporalis muscle and suturing it to selected points on the ipsilateral corner of the mouth). 5 …”

“…Even though MBS patients frequently undergo surgical procedures addressing bone deformities, breathing or feeding difficulties or ophthalmological dysfunction, no curative therapeutic resources exist, so early recognition and intervention could improve the outcome on patient's life expectancy and quality of life. “…Compared with the traditional retrograde temporal muscle procedure, the orthodromic transposition, without changing its direction, prevents production of a softtissue protrusion overlying the zygomatic arch if the muscle is reflected over the arch of the zygoma, altering the dynamics of contraction, which may result in ischemia and decreased contractile strength. 15 However, modified techniques of temporalis muscle transposition have been developed, including the use of slips of temporalis fascia passed to reach the contralateral paramedian plane. 16 Good aesthetic and functional results were achieved.…”

“…Both gracilis muscle free flap transfer and temporalis tendon transfer techniques improve mouth symmetry at rest and during smile, but a better excursion was observed with gracilis free muscle transfer [3]. Many variants of temporalis transposition have been described, using it in an orthodromic or antidromic manner, with or without the use of a fascia sling [4][5][6][7][8]. The present study describes a modification of the orthodromic technique using a cryopreserved fascia lata allograft.…”
