Manganese-Induced Parkinsonism due to Ephedrone Abuse

Abstract: During recent years, a syndrome of hypokinesia, dysarthria, dystonia, and postural impairment, related to intravenous use of a “designer” psychostimulant derived from pseudoephedrine using potassium permanganate as the oxidant, has been observed in drug addicts in several countries in Eastern Europe with some cases also in Western countries. A levodopa unresponsive Parkinsonian syndrome occurs within a few months of abusing the homemade drug mixture containing ephedrone (methcathinone) and manganese. The devel… Show more

“…Environmental risk factors, including prolonged pesticide exposure and lead or manganese intoxication, have been observed to cause parkinsonian symptoms (5)(6)(7)(8). Altered zinc, copper, and iron levels have been found specifically in the substantia nigra of PD patients compared with controls (9).…”

“…Procedures of this type make it impossible to completely remove manganese or chromium ions from the mixture; therefore, compounds containing these elements can enter the human body. Literature data include reports linking the presence of manganese ions delivered in such a manner with Parkinson's disease, possibly as a causative factor [7,8]. Organic synthetic procedures for obtaining ephedrone from ephedrine and its analogues have also been reported [9][10][11].…”
