Marginal zone lymphoma of palatine tonsil with prominent plasmacytic differentiation

Abstract: Rationale:The palatine tonsil is an important component of Waldeyer's ring and a site commonly involved by lymphoma. Interestingly, although it is a site of mucosa-associated lymphoid tissue (MALT), primary MALT lymphoma of the palatine tonsil is rare, especially with prominent plasmacytic differentiation.Patient concerns:A 59-year-old woman presented to the hospital with a 1-month history of odynophagia. The patient had no fever or pruritus during this period and she declared no family history of hematolympho… Show more

“…Symptoms may be acute and distressing in high-grade lymphomas. Low-grade variants such as small lymphocytic lymphoma can manifest as obstructive sleep apnoea or with a short history of only odynophagia (6,7); lingual tonsil hypertrophy and indolent lymphoplasmacytic lymphoma associated with amyloidosis can also present with such symptoms (8,9). The clinical features of dysphagia & odynophagia (72%), cervical lymph node enlargement (60%), and otalgia (7%) noted by Mohammadianpanah (4), were recorded in the present study as well.…”

“…For instance, ITCH controls the stability of critical immune system proteins 41 and acts upstream of B-cell lymphoma 6 (Bcl-6), the main transcription factor involved in coordinating follicular helper T-cell differentiation and immunoglobulin G (IgG) in response to acute viral infections 42 ; WWP2 negatively regulates Toll-like receptor 3 (TLR3)-mediated innate immune response by targeting TIR-domain containing adapter-inducing interferon-β (TRIF) for ubiquitination and degradation 43 . The innate immune system acts as first responder for the detection and clearance of viral infections.…”

“…39 This is consistent with existing literature finding MALT lymphomas within the oropharynx to have the highest survival rates compared to other B-cell and T-cell subtypes, perhaps due to its favorable response to radiation therapy. 19,40 Not surprisingly, T-/ NK-cell and NOS/diffuse malignant lymphomas were associated with significantly lower OS compared to other histologies. T-cell and mixed lymphomas are known to have a poorer prognosis compared to those of B-cell origin due to the former's often more aggressive behavior, 41,42 which our findings confirm.…”
