Marked Underreporting of Pertussis Requiring Hospitalization in Infants as Estimated by Capture–Recapture Methodology, Germany, 2013–2015

Abstract: Our study revealed a high burden of pertussis in infants with marked under-reporting, especially in western federal states where notification was only recently established. Strategies for the prevention of severe pertussis.

“…In Germany, mean annual incidence of pertussis among infants aged ≤3 months was 80 per 100,000 during the past 5 years, while hospitalization rate in those young infants was > 75% [RKI, surveillance data, unpublished]. A recent German capturerecapture study suggests that incidences based on statutory surveillance are substantially underestimated by 39% [5]. Since the introduction of nationwide mandatory pertussis reporting in Germany in 2013, two pertussis-related deaths were notified in infants 2 and 4 months of age [6] [RKI, surveillance data, unpublished].…”

“…Population-based rates were reported in 10 studies from France, Germany, Israel, Norway, Spain, Sweden, the Netherlands, and the UK, including two reports from France and Spain in the gray literature (Figures 2 and 3) [70][71][72][73][74][75][76][77][78][79][80][81]. The pertussis incidence rate in the <3 month age-group ranged from 155.2 per 100,000 population (Netherlands 2005(Netherlands -2007 to 1114.3 per 100,000 population in Spain during a pertussis epidemic [75,76].…”

“…No reliable data are currently available to estimate the extent of pertussis underreporting by these population-based passive surveillance systems. Underreporting is caused by various factors such as lack of physician awareness, rare treatment seeking behavior among mild pertussis cases, lack of biological diagnosis testing capacity, and absence of user-friendly reporting infrastructure (Crowcroft et al, 2018;Schielke et al, 2018). Thus, we recommend the following alternative options.…”
