Hughes–Stovin syndrome (HSS) is a rare potentially fatal vasculitis supposedly belonging to the spectrum of Behçet disease without ocular involvement. HSS tends to play by a temporal pattern, starting with thrombosis and followed by formation of pulmonary aneurysms. Since its mortality can reach 25% of cases, early recognition and appropriate therapy represent the major clinical challenges. We describe a rare case of HSS successfully treated via multidisciplinary management by an endovascular approach and immunosuppressive therapy.

Introduction and importance: Airway obstruction resulting from blood clot formation is observed across various clinical scenarios and is often preceded by hemoptysis. This condition can significantly compromise respiratory function, potentially leading to life-threatening ventilatory distress. Case presentation: In this report, we present a case of acute airway obstruction associated with hemoptysis in an 18-week pregnant woman admitted to the emergency department for acute respiratory distress. Clinical and radiographic evidence strongly suggested the presence of an endobronchial blood clot causing focal airway obstruction. Diagnosis was confirmed through direct endoscopic evaluation. Clinical discussion: Initial attempts to remove the obstructing clot from the airway involved lavage, aspiration, and forceps extraction by using a bronchoscope. In cases in which these measures proved ineffective, other management strategies include rigid bronchoscopy, embolization, and surgical resection. Conclusion: Central airway obstruction is a critical condition caused by numerous factors such as tumors or blood clots. Treatment focuses on securing the airway, ensuring breathing, and using tools such as bronchoscopy for diagnosis and treatment. Surgery is considered a last resort when other methods are ineffective.

Behçet's disease (BD) is a rare disease, characterized by chronic perivascular inflammation, recurrent oral and genital ulcers, ocular inflammation and multisystemic involvement due to vasculitis. It is more common in countries along the old Silk Road such as Turkey, Greece, and Israel. The etiology is unknown, while epidemiologic data point to the role of both genetic and environmental factors. 1,2 BD usually presents at third to fourth decades and is rare in children. Vasculitis in BD can affect both arteries and veins; however, venous involvement is more common. Although the most common arterial involvement is pulmonary artery aneurysm (PAA), it occurs very rarely in BD. In a retrospective analysis, the prevalence of pulmonary artery involvement including PAA and pulmonary artery thrombosis was found to be 2%. 3 To our knowledge, there is no data about the prevalence of PAA in children with BD. Patients with PAA usually present with hemoptysis and carry a high risk of mortality. 4 It should be kept in mind that some life-threatening features in the pediatric population may occur earlier than mucocutaneous findings. In this article, we report an adolescent diagnosed with BD to emphasize the importance of vascular etiologies such as BD in the differential diagnosis of hemoptysis even in the pediatric age group.
