Maternal and neonatal outcomes following “natural” caesarean section

Abstract: Background “Natural” caesarean section (NCS) is a technique in which the obstetrician replicates the mechanisms of normal parturition at the time of caesarean delivery.1 NCS has the potential to improve the birth experience for the mother and might also reduce the incidence of neonatal respiratory morbidity associated with caesarean delivery. There is no published quantitative data on the safety of NCS to validate the technique, only anecdotal evidence from current advocates. Aims To compare outcomes for wom… Show more

“…Our findings are similar to that of previous studies and case series, which identified no difference in maternal outcomes and most neonatal outcomes. 3,[5][6][7][8]11 Similarly to Armbrust et al, no difference in operating time was also noted between groups. 5 To our knowledge, no previous studies have described the difference in time for skin-toskin contact between methods, with Armbrust et al only identifying that earlier skin-to-skin contact was achieved in 72% of cases and Bronsgeest et al only identifying that the median time to first skin-to-skin contact was 3 minutes with the family-centered method.…”

“…Also known as a gentle, natural, skin-to-skin, or Charité cesarean, limited studies have evaluated the family-centered cesarean alone or compared it to the traditional cesarean delivery. 3,[5][6][7][8][9][10][11] These previous studies and case series have identified improved patient satisfaction 5,10 and higher breastfeeding rates, 5,8,10 as well as no difference in maternal outcomes or complication rates with family-centered cesarean deliveries. 3,[5][6][7][8]11 Most studies have also identified or suggested no difference in neonatal outcomes, 5,7,8,11 with one identifying a decrease in neonatal intensive care unit (NICU) admissions and suspected neonatal infection.…”

“…The previous studies and case series have identified an improved patient satisfaction [4,9] and higher breastfeeding rates, [4,6,9] moreover there is no difference in maternal outcomes or complication rates with family-centered cesarean deliveries [2,[4][5][6][7]. In contrast to the majority of research [3,10,13], one found a decrease in admissions to newborn critical care units and suspected neonatal infections [8]. Breaking the sterile barrier has possible dangers in addition to the caesarean section, such as a higher risk of surgical site infections [14,15].…”

“…In order to enhance the birthing process, numerous hospitals have adopted a family-centered caesarean delivery strategy that mimics a vaginal delivery. Only a few studies have examined the family-centered caesarean separately or contrasted it to the standard caesarean delivery, which is referred described as a gentle, natural, skin-to-skin, or Charité caesarean [2,[4][5][6][7][8][9][10]. The skin-to-skin caesarean sections differ significantly from standard caesarean sections in that the surgical drape is dropped before the newborn is carefully delivered, facing the parents so they may see the birth process.…”
