Maternal and neonatal peripartum factors associated with late initiation of breast feeding in Bangladesh: a secondary analysis

Abstract: ObjectivesLate initiation of breast feeding (LIBF) is associated with increased neonatal mortality and morbidity. This study aimed to assess the association between intrapartum, early postpartum and neonatal factors, and LIBF in Bangladesh.Design, setting and participantsIn this analysis, we used data from the mothers participating in a cluster-randomised controlled trial (Rang-Din Nutrition Study) conducted in rural northwest Bangladesh. Mothers (n=3594) were interviewed about the time of initiation of breast… Show more

“…Elective caesarean section, administration of antibiotics, respiratory distress in babies after birth, and maternal BMI <18 kg/m 2 were signi cant predictors of early initiation of breastfeeding. Findings that women who birthed by elective caesarean section were less likely to initiate early breastfeeding is consistent with studies conducted in Canada (Hobbs et al, 2016) and Bangladesh (Roy et al, 2022). In contrast, Zanardo et al (2010) reported that early initiation of breastfeeding is less likely after emergency caesarean section.…”

“…Similar results were observed in babies born in Bangladesh (Roy et al, 2022) who had asphyxia. In these scenarios, mothers and babies may be separated due to medical interventions and breastfeeding may not be recommended until recovery (Roy et al, 2022). We also found that women who were administered antibiotics were less likely to initiate breastfeeding early.…”

“…There is a need for Sri Lankan health services to ensure that healthcare staff are competent and compliant in not recommending breastmilk supplements unless medically necessary (BFHI Step 6) and promoting mother-baby skin-to-skin contact (BFHI Step 4). Universal compliance with Step 4 of BFHI may be particularly challenging for healthcare staff, as mother-baby skin-to-skin contact is often delayed due to postoperative care following caesarean section [11,32].…”

“…Several studies have reported that the mode of delivery is one of the main determinants of EIBF status in newborns and that cesarean delivery delays the initiation of breastfeeding (21,(31)(32)(33). A Baby-Friendly Hospital in Turkey showed that the rates of initiation of breastfeeding in babies born by cesarean section were significantly lower, up to the 4th hour, compared with babies born vaginally (31,32).…”

“…A Baby-Friendly Hospital in Turkey showed that the rates of initiation of breastfeeding in babies born by cesarean section were significantly lower, up to the 4th hour, compared with babies born vaginally (31,32). Generally, delay in SSC, fatigue, and then delay in the start of breastfeeding are seen due to the sedative effects of anesthesia and additional procedures and problems that may occur to the mother and the baby due to cesarean intervention (21,31,33).…”
