Maternal Skin Allograft for Cicatricial Ectropion in Congenital Icthyosis

Abstract: A 6-month-old infant with congenital icthyosis was brought to the authors with bilateral cicatricial ectropion of the upper and lower eyelid with resultant exposure keratopathy. Skin autograft was not possible due to generalized skin involvement, leading to unavailability of a donor site. A maternal skin allograft was performed for both eyelids of each eye. Prior human leukocyte antigen typing was not done, and postoperative immunosuppressive agents were not given. At 10 months' follow-up, the graft was well a… Show more

“…There are reports in the literature on the surgical management of cicatricial lower eyelid ectropion with skin grafting in cases of burns, 8 congenital and acquired skin abnormalities, 9,10 prior blepharoplasties, 11 postradiation scarring, 12 and excision of eyelid tumors. [13][14][15] Actinic skin change as an indication for repair is rarely specified.…”

“…Shimkin90 reported, with less than 3 months follow-up, the use of forearm skin from the mother for a 13-month child with ectropion of three eyelids. More recently, in India, Das et al 91 reported the use of maternal bilateral postauricular skin for a 6-month-old baby with cicatricial ectropion due to ichthyosis. Maternal testing for HIV, hepatitis B and C infection was carried out but not for human leucocyte antigen typing.…”

“…4,5 Extraordinary approaches using mucous membrane graft or maternal skin allograft for correcting cicatricial ectropion has also been reported. 8,9 In our case, the postauricular skin grafts were harvested for upper eyelid reconstruction. After suturing the graft to the defect an additional upper eyelid lateral tarsal strip procedure was performed to maintain exact apposition of the upper eyelid margin.…”
