, DO ature benign cystic teratomas are among the most common ovarian neoplasms, constituting 95% of germ cell ovarian tumors and 15% to 54% of all ovarian tumors. Ovarian teratomas are found bilaterally in 10% to 13% of cases. 1 Despite the frequency of this neoplasm, the appearance of multiple synchronous ovarian teratomas within the same ovary is a rare occurrence. We were unable to find any prior sonographic descriptions of this entity or any previous reports in which a preoperative diagnosis was made. We report a case of bilateral benign cystic teratomas, including multiple teratomas in the right ovary, diagnosed preoperatively on the basis of transvaginal ultrasonography.
Case ReportA 49-year-old gravida 3, para 4, obese woman had a recent history of dysfunctional uterine bleeding and was subsequently referred for pelvic ultrasonographic examination. Transabdominal, transvaginal, and color Doppler sonography demonstrated bilaterally enlarged ovaries containing multiple complex cystic masses. The right ovary measured 10.0 × 6.0 × 5.0 cm and contained 4 discrete masses, ranging in size from 6.0 to 0.7 cm (Fig. 1). Each mass had a dermoid plug with acoustic shadowing, and 3 of the masses had brightly reflective internal septations. Each mass within the right ovary was discretely separated from the other masses by intervening ovarian tissue. Arterial and venous color Doppler signals were present within both ovaries. The left ovary measured 7.5 × 5.3 × 4.8 cm and was occupied by a 5.3-cm complex cystic mass containing a hyperechoic, shadowing dermoid plug.
