Evaluation of mean platelet volume and its ratio over platelet count in children with obstructive sleep apnea syndromeTıkayıcı uyku apne sendromu olan çocuklarda ortalama trombosit hacmi ve ortalama trombosit hacminin trombosit sayısına oranının değerlendirilmesi ABSTRACTObjectives: This study aims to determine the association between obstructive sleep apnea syndrome (OSAS) caused by adenotonsillar hypertrophy and mean platelet volume (MPV) and MPV/platelet count ratio in children, and to evaluate the impact of adenotonsillectomy on these two parameters. Patients and Methods:This prospective study consisted of 73 child patients (38 boys, 35 girls; mean age 8.6±3.3 years, range 2 to 17 years) with chronic adenotonsillar hypertrophy who applied to Gaziosmanpaşa University Faculty of Medicine, Ear, Nose and Throat policlinics due to witnessed apnea and snoring in January 2011 and January 2013. Also, 56 age and sex matched pediatric patients (28 boys, 26 girls; mean age 8.0±3.2 years; range 3 to 13 years) who admitted to our clinic due to reasons besides OSAS were included in the study as control group. Preoperative and postoperative third month hemoglobin, white blood cell, thrombocyte count, and MPV values of the patients were recorded. Results:Although ratio of MPV/platelet count was higher in patient group than in control group, the difference was not statistically significant. Mean platelet volume level was 7.68±1.07 fL in patient group and 7.21±0.84 fL in control group. Preoperative MPV level in patient group was significantly higher than that in control group (p<0.05). A statistically significant decrease was detected in postoperative third month MPV level and platelet count compared with preoperative MPV and platelet count (7.68±1.07 fL and 7.17±0.97 fL, respectively; p<0.05). Conclusion:High MPV and MPV/platelet count in children with adenotonsillar hypertrophy may be an important risk factor for cardiopulmonary and cerebrovascular morbidities which may develop both in childhood and in adulthood. Adenotonsillectomy may reduce this risk significantly.Keywords: Adenotonsillectomy; mean platelet volume; pediatric obstructive sleep apnea. Background: Adenoid hypertrophy (AH) leads to chronic upper airway obstruction. Upper airway obstruction may cause pulmonary hypertension and right ventricle dysfunction. Mean platelet volume (MPV) may be used as a marker of platelet activation and it is shown that MPV is related to cardiovascular disorders. We investigated in our study if MPV is correlated with adenoid size and can be used as an indicator of obstruction due to adenoid hypertrophy. Methods: 95 children with AH and 99 healthy controls were enrolled in the study. White blood cell, hemoglobin, platelet, Mean platelet volume (MPV) levels were measured before adenoidectomy. Lateral cephalometric graphs were obtained from all patients to measure adenoid/nasopharynx ratio (A/N). The distance between the outermost point of convexity of adenoid shadow and basiocciput was divided to the distance between sphenobasiocciput and posterior end of the hard palate. A/N ratio and MPV levels have been correlated by using Pearson correlation test. Results: There were 45 male and 50 female patients between the ages of 2 and 14 years (mean age: 6.47 ± 2.44). There were no significant differences between children with AH and controls regarding white blood cell, hemoglobin, platelet, MPV and A/N ratio. We didn't detect any correlation between MPV levels and A/N ratio. Conclusion: MPV level was in normal limits in AH children and there was no significant relation between MPV and adenoid severity.

Objectives: To explore the relationships between mean platelet volume (MPV), neutrophil to lymphocyte ratio (NLR) and the size of adenoid hypertrophy(AH) and the effect of adenoidectomy on MPV levels and NLR values. Subjects and methods:In total, 132 (66 males, 66 females; average age 7.10 ± 3.62 years; range, 3-18 years) children who underwent adenoidectomy were included in the study. The patients were divided into two groups according to the size of adenoid tissue volume. Group A was consist of 72 patients with grade 3 AH, while group B was consist of 60 patients with grade 4 AH. Preoperative MPV levels and NLR values of group A and B were compared with measurements in the postoperative third month. A p-value <0.05 was considered to reflect statistical significance. Results:In group A, mean preoperative MPV values were 7.80 ± 1.14 fL and mean postoperative MPV values were 8.06 ± 1.02 fL, respectively. In group B, mean preoperative MPV values were 7.44 ± 1.86 fL and mean postoperative MPV values were 7.68 ± 1.14 fL, respectively. No statistically significant difference in mean MPV values was observed between pre-and postoperatively in both groups A and B (p=0.190, p=0.259, respectively). In group A, the preoperative values of NLR were 1.62 ± 1.10 and the postoperative values of NLR were 1.34 ± 2.86, respectively. In group B, the preoperative values of NLR were 1.38 ± 0.92 and the postoperative values of NLR were 1.40 ± 1.12, respectively. No statistically significant difference in mean NLR values was observed between pre-and postoperatively in both groups A and B (p=0.137, p=0.191, respectively). Conclusion:Adenoidectomy did not decrease MPV levels and NLR values in patients with grade 3 and 4 AH. Future randomized studies should explore the relationship between the MPV levels, NLR values and all of the size of AH in larger numbers of patients.
