Measured outcomes of chronic care programs for older adults: a systematic review

Abstract: BackgroundWagner’s Chronic Care Model (CCM), as well as the expanded version (ECCM) developed by Barr and colleagues, have been widely adopted as frameworks for prevention and management of chronic disease.
