Measurement-based management of mental health quality and access in VHA: SAIL mental health domain.

Abstract: We outline the development of a Mental Health Domain to track accessibility and quality of mental health care in the United States Veterans Health Administration (VHA) as part of a broad-based performance measurement system. This domain adds an important element to national performance improvement efforts by targeting regional and facility leadership and providing them a concise yet comprehensive measure to identify facilities facing challenges in their mental health programs. We present the conceptual framewo… Show more

“…Identifying mechanisms for retention and reengagement in mental health and general medical care for people with serious mental illness outside VHA is critical for improving the health of this population. Within VHA, efforts to improve care continuity for veterans with serious mental illness include identifying those at risk of loss to care (32), developing strategies to improve care retention, and sustaining the SMI Re-Engage program.…”

“…More research is needed to examine the utility of using symptom dynamic indices to characterize and predict comorbidity in larger, more diverse samples. These findings provide useful implications for clinical practice, particularly related to measurement-based care initiatives that call for increased symptom monitoring to increase standards of care (e.g., within national healthcare systems; Lemke et al, 2017). However, what to do with those measurements and how to best incorporate them into clinical practice requires continued discussion.…”

“…Furthermore, few studies have compared these dynamic metrics to determine their relative contributions to characterizing symptomatology. Symptom dynamic research corresponds to measurement-based care initiatives that call for repeated symptom monitoring to increase standards of care (e.g., within national healthcare systems; Lemke et al, 2017).…”

“…Data to knowledge involves the process of identifying and understanding determinants of health and health care quality gaps and evaluating interventions that might reduce or eliminate these gaps. VA national program offices such as the Office of Mental Health and Suicide Prevention (OMHSP) have used longitudinal electronic health record data to identify and understand the drivers of suicide and opioid use disorder risk, assess adherence to evidence‐based guidelines, track variation in practice, monitor performance, and reward improvement 29 . Data to knowledge also involves careful evaluation of interventions that are effective in real‐world practice.…”
