Measuring health-related quality of life in patients with conservatively managed stage 5 chronic kidney disease: limitations of the Medical Outcomes Study Short Form 36: SF-36

Abstract: Purpose Chronic kidney disease (CKD) negatively affects health-related quality of life (HRQoL), which is often measured using the Medical Outcomes Study Short Form 36 (SF-36) questionnaire. However, the adequacy of SF-36 in this population has not been reported. We aimed to determine floor and ceiling effects and responsiveness to change of SF-36 in patients with conservatively managed stage 5 CKD.MethodsSF-36 data were collected prospectively. Floor and ceiling effects were estimated for each SF-36 scale and … Show more

“…The proportion of patients experiencing a change exceeding the minimal clinically important difference (MCID) threshold was determined for each SF‐36 subscale at 12 months posttransplant. The threshold values reflect the minimum score change that is clinically meaningful for patients and were obtained from a study of patients with stage 5 chronic kidney disease …”

“…Chronic-kidney-disease-specific instruments are PROMs that are tailored to the specific symptom burden and disease experience by patients with CKD (see Table 2 ) ( 22 , 116 , 127 , 159 ). These instruments are often very specific, thus avoiding the ceiling or floor effects observed when using generic instruments ( 127 , 132 ). The specificity of the questions, however, usually precludes its use in other populations thus limiting generalizability of results ( 127 , 133 ).…”

“…Many studies fail to mention the existence of missing and the methods of handing it [2,3]. Others merely alluded to the default method or typically discard of samples with missing value to obtain a complete dataset [4]. These methods, known as listwise deletion or complete case analysis, reduce even more sample size available for analysis [5].…”
