Meat balls: a pathognomonic ultrasound and computed tomography finding in mature cystic teratoma

Abstract: One of the commonest germ cell tumours are mature cystic ovarian teratomas. Although the imaging features of teratomas are highly variable, the sonographic and CT identification of fatty and calcific components are fairly suggestive. The case presented, however, is of an unusual, yet pathognomonic CT and ultrasound imaging appearance of an ovarian teratoma that was due to floating balls.

“…Differences in the physical and thermal properties of the material being deposited around each nidus create discrete masses rather than an amorphous mass [38]. Furthermore, the specific gravity of the globules is low because of fatty composition; therefore, they float in the most gravity-independent position within the cyst fluid or in the interface when there is a fat-fluid level [33]. Some spherules may have a 2–3-mm-thick outer sebaceous shell and a less dense central core [31].…”

“…In addition, cystic teratoma are typically associated with poor sound transmission and an uneven echo. Sometimes, hyperechoic floating fat balls or globules can be observed inside the teratoma (28).…”

“…The high density Rokitansky node consisting of hair and other components can swim in the middle of the mass and calci ication is frequently present [26]. The sign of loating balls is a striking and rare inding of MCT and consists of several small spherical structures within the cyst [27].…”
