Mechanical Circulatory Support—Results, Developments and Trends

Abstract: The implantation of ventricular assist devices (VADs) is a valuable option in patients with end-stage heart failure. The number of VAD implantations is growing worldwide. Between July 1987 and July 2010, we implanted 1,598 VADs in 1,455 patients. The majority were male (81.0%), and their mean age was 49.4 years (range 0.3-82 years). Indications for implantation were: cardiomyopathy (n = 1,074), post-cardiotomy heart failure (n = 282), acute myocardial infarction (n = 83), graft failure after heart transplantat… Show more

“…Various volume-displacement total artificial hearts (TAHs) have been developed over the past 50 years. 3 These VADs are robust; they have only one moving part and do not have valves or contain flexible membranes, so mechanical wear is minimized. 2 Continuous-flow implantable VADs have been in development since the 1980s and are small enough to implant in a greater percentage of patients.…”

“…Infection rates up to 60% have been reported . Infection is also closely related to survival of the patients after heart transplantation . Different efforts are undertaken to minimize the risk for a driveline infection, for example, skin buttons .…”

“…An increasing and diverse data set would suggest that VAD therapy produces superior survival results and improvement in quality of life (QoL) compared with medical therapy. Similar results have been appreciated for those patients who are inotrope dependent waiting for a heart transplant 1 2. VADs can be used to support the left ventricle assist device (LVAD), right ventricle assist device or both ventricles (BVAD) in patients with severe heart failure or cardiomyopathies.…”
