Mechanical Thrombectomy in Basilar Artery Thrombosis: Technical Advances and Safety in a 10-Year Experience

Abstract: Purpose Multiple endovascular devices have been used for mechanical thrombectomy (MT) in basilar artery occlusion (BAO) for [10 years. Based on a single-center experience during the course of one decade, we present data on safety and efficacy of previous MT devices compared with modern stent retriever and suction thrombectomy. Results Recanalization rates of 95 % were high in both groups. Procedure-related dissection (n = 5) and subarachnoid hemorrhage (n = 9) occurred in group 1 but not in group 2 (p \ 0.016)… Show more

“…[38][39][40] Experience at the Karolinska Hospital showed a 57% rate of good functional outcome (95% CI 37% to 75%), and of 73% (95% CI 50% to 89%) when there were no signs of acute infarction prior to treatment, with about 21% mortality. 41 Recanalization rates over 75% were reported with new generation devices 37,42 as well as with older generation devices in the MERCI and multi-MERCI trials but with lower benefit. 43 A previous prospective registry, the Basilar Artery International Cooperation Study (BASICS) could not demonstrate superiority of endovascular therapy against IVT; however, it employed mostly oldergeneration devices.…”

“…The use of retrievable stents for the treatment of endovascular ischemic stroke increased recanalization rates up to 80%-94% [11,[15][16][17][18]. On the other hand, there is still a diminished literature with only few studies on mechanical thrombectomy with stent retrievers in patients with acute BAO [7,18].…”

“…On the other hand, there is still a diminished literature with only few studies on mechanical thrombectomy with stent retrievers in patients with acute BAO [7,18]. In Baek et al [8] study, 25 patients with BAO were treated within 8 hours of stroke onset with mechanical thrombectomy using Solitaire stent, reported a 88% rate of survival at 3 months (22/25), and more than half of the survivors (55%, 12/22) had a good outcome.…”

“…Une autre étude du même centre que RECOST portant sur 31 patients consécutifs avec occlusion de l'artère basilaire traitée avec le système Solitaire FR retrouvait une bonne corrélation entre un score en diffusion et une évolution clinique favorable [35]. L'essai en cours BASICS a pour but d'évaluer la TM dans les 6 heures.…”
