Mechanisms by which epinephrine augments cerebral and myocardial perfusion during cardiopulmonary resuscitation in dogs.

Abstract: The goals of this study were to quantify the effects of epinephrine on myocardial and cerebral blood flow during conventional cardiopulmonary resuscitation (CPR) and CPR with simultaneous chest compression-ventilation and to test the hypothesis that epinephrine would improve myocardial and cerebral blood flow by preventing collapse of intrathoracic arteries and by vasoconstricting other vascular beds, thereby increasing perfusion pressures. Cerebral and myocardial blood flow were measured by the radiolabeled m… Show more

“…After 3 min, one of six methods of CPR was performed for 17 min including: manual American Heart Association Standard [13], High Impulse Compression [14], Interposed Abdominal Compression [15], Thumper @ performed American Heart Association Standard [13], Simultaneous Compression and Ventilation [6], or Vest CPR [8]. Each animal received only one form of CPR.…”

“…During cardiopulmonary arrest, cen tralization of blood from peripheral vessels occurs and is enhanced by the administration of epineph rine (Michael et al, 1984;Schleien et aI., 1986). Cephalic muscle and skin blood flow (extracranial blood flow) decreases with the administration of epinephrine (Michael et 1986).…”

“…Vasopressors are thought to prevent arterial collapse as intrathoracic pressure is raised by chest compressions (Michael et al 1984). Aortic diastolic pressure, an important determinant of coronary blood-flow is increased (Michael et al 1984;Brown and Werman 1990). Maintenance of coronary blood-flow is the most important factor in enabling return of spontaneous circulation (ROSC) or successful defibrillation (Kern et al 1988;Paradis et al 1990;Paradis et al 1991;Babbs et al 2001;Zhong and Dorian 2005) It is interesting to note that one historical resuscitation method was to throw cold water at the face of the victim.…”
