Mechanisms of Neuropathic Pain

Abstract: Neuropathic pain refers to pain that originates from pathology of the nervous system. Diabetes, infection (herpes zoster), nerve compression, nerve trauma, "channelopathies," and autoimmune disease are examples of diseases that may cause neuropathic pain. The development of both animal models and newer pharmacological strategies has led to an explosion of interest in the underlying mechanisms. Neuropathic pain reflects both peripheral and central sensitization mechanisms. Abnormal signals arise not only from i… Show more

“…These two variables represent psychosocial aspects of chronic back pain, thus social interactions with adaptation to the sick role or a negative attitude and belief toward possible recovery may influence the prognosis. Central sensitization can also contribute to the maintenance of pain and disability in patients with long-lasting low back pain [32].…”

“…Another possibility is the spontaneous activities in the uninjured L4 primary afferent fibers adjacent to the injured fibers [34] . The third possibility is the central mechanism, such as microglial activation in the spinal cord, disinhibition of the descending inhibition system and activation of descending facilitatory system [2,35] .…”

“…The mechanisms underlying neuropathic pain remain still elusive [2,3] . There has been evidence that neuropathic pain is related to the sensitization of dorsal horn neurons.…”

“…Pain following traumatic nerve injuries has been reported to be associated with poor outcomes 5,15,[59][60][61] . Brachial plexus injury is often associated with more severe pain and is a significant predictor of disability.…”
