Mechanistic implication of decreased plasma atrial natriuretic peptide level for transient rise in the atrial capture threshold early after ICD or CRT-D implantation

Abstract: Atrial degenerative change characterized by lower ANP/BNP ratio was associated with the occurrence of TRACT in patients with heart failure. TRVCT could also occur, but it required a longer recovery time than TRACT.

“…Yoshida et al also proposed the original index ANP/BNP ratio, which may be more sensitive to a heart condition and better reflects atrial integrity than ANP or BNP alone. Patients with more severe HF (higher BNP) and more advanced atrial fibrosis (lower ANP) have a much lower ANP/BNP ratio than those without these conditions [106]. However, this interpretation of the ANP/BNP ratio needs validation in future studies, and assessment of the MR-proANP /BNP ratio is also of interest with respect to atrial remodeling in patients with HF and AF.…”

“…The ANP/BNP ratio predicted left atrial reverse remodeling after catheter ablation for persistent AF, and a transient rise in the atrial capture threshold, which is associated with atrial myocardial damage, after implanting a high-voltage device. 42,45 The ANP/BNP ratio has also been shown to be correlated with left atrial fibrosis in the maze procedure. 46 Thus, the ANP/BNP ratio is thought to be a more precise measure for predicting left atrial remodeling than ANP alone.…”

“…Previous studies also used the ANP/BNP ratio as a clinical marker. The ANP/BNP ratio predicted left atrial reverse remodeling after catheter ablation for persistent AF, and a transient rise in the atrial capture threshold, which is associated with atrial myocardial damage, after implanting a high-voltage device 42,45 . The ANP/BNP ratio has also been shown to be correlated with left atrial fibrosis in the maze procedure 46 .…”

“…Levels of plasma ANP and BNP were measured within 1 month before catheter ablation. Plasma ANP level was measured with a chemiluminescent enzyme immunoassay (CLEIA), and the BNP level was also measured with a chemiluminescent immunoassay (CLIA) (BML, Inc., Tokyo, Japan) [ 8 , 9 ]. After ablation, these biomarkers were systematically measured annually until recurrence of AF was documented and a redo procedure was performed.…”
