Medial Meniscal Allograft Transplantation: The Bone Plug Technique

Abstract: The medial meniscus is crucial for knee homeostasis. Treating patients who have undergone a subtotal or total meniscectomy, or equivalent irreparable tear pattern, can be extremely challenging, especially in young, active patients. The importance of meniscal preservation has been reported by several authors. Meniscal repair is now widely accepted as the first surgical option for treating medial meniscal tears. Moreover, current guidelines recommend preserving as much meniscal tissue as possible. Treating a sym… Show more

“…For press-fit fixation, the bone block, which is attached to the meniscal allograft roots, is pushed into the trough [ 19 ]. In contrast, for medial MAT, two small bone plugs, which are attached to the anterior and posterior root of the medial meniscal allograft, are inserted into the respective tibial bone tunnel and fixed in a transtibial pull-out fashion [ 62 ]. Additional soft tissue fixation is performed utilizing inside-out, outside-in or all-inside meniscal sutures, as appropriate.…”

“…Most commonly, separate anterior and posterior bone plug fixation with transtibial suture fixation or a bone-bridge (i.e., trough or bone-in-slot) techniques with interference fit/ interference screw fixation are utilized. 61,62 For peripheral fixation of the meniscal transplant, a combination of allinside, inside-out, and outside-in approaches are utilized for secure transplantation of a size-matched allograft.…”
