Background: The mediastinum is the mid portion of the thoracic cavity, further divided into the anterior, middle, and posterior mediastinum. The numerous organs within the mediastinum make it prone to the development of various lesions. About half of all mediastinal masses occur in the anterior compartment. The most common are thymoma, lymphoma and mature teratoma. An accurate histopathological diagnosis is required for the management of mediastinal lesions. Methods: This is a 2-year prospective study conducted in the Department of Pathology on Biopsies and Resected Specimens of Anterior Mediastinal Masses (AMM) in a tertiary care hospital from September2020-September2022. Sections were studied after staining with hematoxylin and eosin(H&E). Immunohistochemical (IHC) stains were done wherever required. Result: The present study included 58 cases of AMM. The age of the patients ranged from 2-77 years. Males constituted 68.90 % of cases. The male-to-female ratio was 5:2.2. Cough was the most common presenting symptom. Thymic pathology was detected in 56.8% of cases, of which thymoma is the most common. Thymoma presented with Myasthenia Gravis (M.G.) in 2 cases. Lymphoma constituted 12% of cases. Metastatic carcinoma in 12%, Extragonadal Germ Cell Tumor (EGCT) in 8.6% and lymphangioma in 3.4% cases. Lipoma, sarcoidosis and myofibroblastic tumor each constitute 1.72% cases. Conclusion: The present study describes the various lesions of the anterior compartment.

Согласно статистике, количество женщин с диагностированным раком молочной железы в мире на 2020 г. составило 2,3 млн чел. Количество смертей от рака молочной железы составило примерно 690 000 (6,9 %) случаев. Около 70 % умерших женщин имели метастазы. Одним из основных способов метастазирования рака молочной железы считается лимфогенное распространение, что делает крайне необходимым понимание топографо-анатомических особенностей лимфатического русла молочной железы, что может стать ключом к пониманию путей метастазирования, позволит более точно определить объем оперативного вмешательства и прогнозировать вероятность возникновения отдаленных метастазов. В рамках данного литературного обзора подробно разобраны исторические аспекты изучения анатомии глубоких и поверхностных лимфатических сосудов молочной железы, рассмотрены особенности их топографии по данным современных исследований и представлены различные варианты лимфогенного метастазирования на примере клинических случаев. Обсуждаются некоторые результаты исследования, проведенного авторами данной статьи, в котором методами цветной лимфографии, классической гистологии и иммуногистохимии с использованием антител к подопланину (D2-40), гладкомышечному актину (SMA), панцитокератину AE1/AE3 и транскрипционному фактору GATA3, изучено на макро- и микроскопическом уровне подареолярное лимфатическое сплетение Саппея, показана его связь с лимфатической системой молочной железы и выявлены признаки, подтверждающие участие подареолярного лимфатического сплетения в процессе лимфогенного метастазирования при раке молочной железы с метастазами в подмышечных лимфатических узлах. Для подготовки обзора проведен поиск литературы по базам данных Scopus, Web of Science, Medline, PubMed, CyberLeninka, РИНЦ и CNKI. При анализе использованы источники, индексируемые в базах данных Scopus и Web of Science (78 %), РИНЦ и CNKI (22 %); 35 % работ опубликовано за последние 5 лет. Использовано 37 источников для написания данного литературного обзора.

Role of EBUS-TBNA in Non-Neoplastic Mediastinal Lymphadenopathy: Review of Literature

Mediastinal lymphadenopathy is a condition in which one or more mediastinal lymph nodes are enlarged for malignant or benign causes, generally more than 10 mm. For a long time, the only way to approach the mediastinum was surgery, while in last decades endoscopic techniques gained their role in neoplastic diseases. At the present time, EBUS is the technique of choice for studying the mediastinum in the suspicion of cancer, while there are not strong indications in guidelines for the study of benign mediastinal lymphadenopathy. We reviewed the literature, looking for evidence of the role of EBUS in the diagnostics of non-neoplastic mediastinal lymphadenopathy, with special regard for granulomatous disease, both infectious and non-infectious. EBUS is a reliable alternative to surgery in non-neoplastic mediastinal lymphadenopathy, even if more evidence is needed for granulomatous diseases other than tuberculosis and sarcoidosis.
